For child or adolescent clients, please download and complete the Child Therapy Registration and Disclosure Form. Please note that if the child's parents are divorced and share legal medical decision-making, both parents must sign their consent.
A note about health insurance: While I no longer accept direct, in-network payment from managed care insurance companies, I can write a "superbill" for you to submit to your insurance company yourself. This form contains all the information your insurance company needs to know, if you have out-of-network benefits for which they will reimburse you directly. Please check the benefits on your specific health insurance policy.
